Locker Request Form

If you would like a locker, please click on this link Locker Request Form. 

If you have an area you would prefer your locker be located, please specify that on the Google Form. There are no guarantees that your locker will be located in that area, but an attempt will be made to accommodate your request. 

Once it is assigned, your locker location and combo will show on your LSHS schedule - log into the Student Portal and go to Documents and pull up your LSHS schedule. The locker information will be at the bottom of the schedule.
