Locker Request Form

LSHS student lockers are available upon request!  

If you would like a locker, please complete this Locker Request Form or stop into the Student Services Office 

  • Lockers are free.  There is no cost for a student locker!

  • You may request one locker for the school year. 

  • Lockers are assigned for the current school year only. You must empty your locker at the end of the school year.  

Once assigned, your locker number and combination can be found in your Infinite Campus student portal under More > Lockers
